Changes in memory and thinking can be a normal part of ageing, but sometimes they can be a sign that further assessment may be helpful.
For a limited time, Parkhill Medical Centre is offering Dementia Risk Assessments at NO COST until 30 September 2026.
If you’ve noticed changes in your memory, concentration or everyday functioning or you’re concerned about a family member, this can be a good opportunity to speak with your GP.
Why is it important to check changes in memory?
Everyone forgets things from time to time. Forgetting a name, misplacing your keys or walking into a room and forgetting why you’re there doesn’t necessarily mean you have dementia.
However, ongoing or noticeable changes in memory, thinking or everyday activities are worth discussing with your GP.
There are many possible reasons for changes in memory and concentration, and not all of them are related to dementia. Your GP can help explore your concerns and determine whether any further assessment or investigation may be appropriate.

Who might benefit from a dementia risk assessment?
You may wish to speak with your GP if you or someone close to you has noticed:
- Increasing forgetfulness
- Difficulty remembering recent conversations or events
- Frequently forgetting appointments or important information
- Difficulty concentrating or following conversations
- Finding familiar everyday tasks more difficult
- Changes in problem-solving or decision-making
- Concerns about changes in behaviour or personality
- A family member or friend has expressed concerns about your memory
You don’t need to wait until memory problems are significantly affecting your daily life before talking to your GP.
What happens during an assessment?
Your GP will take the time to understand your concerns and discuss any changes you’ve noticed.
Depending on your individual circumstances, your GP may discuss:
Your medical history
Your health, medications and any factors that may be affecting your memory or thinking.
Your concerns and symptoms
When the changes started, how they have progressed and how they may be affecting your everyday life.
Memory and thinking
Your GP may use a brief cognitive assessment where appropriate.
Next steps
Your GP can discuss whether further investigation, monitoring, support or referral may be appropriate.
An assessment does not automatically mean that you have dementia. Memory problems can have many different causes, which is why a thorough discussion with your GP is important.
