• 24 hours’ notice is required for cancellations or rescheduling. Deposits are non-refundable for no-shows or late cancellations.
• All new clients must complete a medical history and consent form prior to treatment. Consultation is required before your first injectable procedure.
• Clients must be 18 years or older. ID may be required.
• Please arrive on time. Late arrivals over 10 minutes may result in a shortened or forfeited appointment
